This form must be handed into reception before an appointment can be made with the nurse.
Vaccinations take approximately two weeks to produce a protective effect in your body, so please ensure that you make an appointment at least two weeks before your travel date.
It is your responsibility to ensure you have a suitable appointment.
Alternatively appointments can be made at specialist travel clinics these addresses and telephone numbers can be found overleaf. Please note these clinics do charge for injection - again see form for details.
Please complete the enclosed travel form and return it to the Surgery reception as soon as possible. Vaccinations generally take around 2 weeks to produce a protective effect in your body. Some vaccinations are given over the course of several months.
