Registering at our practice

We accept patients from Peacehaven, Telscombe Cliffs and Saltdean.

Identification

All new patients are required to present identification upon registration. You will need to provide two forms of identification; one with your address (eg:- utility bill) and one photo ID.

We are able to accept the following documents for identification

• Birth certificate
• Marriage Certificate
• Medical card
• Driving Licence
• Passport
• Local Authority Rent card
• Paid Utility Bills
• Bank Cards / Statements
• National Insurance Number card
• Wage Slip
• Evidence of Benefit Entitlement (letter about benefits, exemption certificate etc.)
